We reproduce this data as-is. We did not conduct the surveys, calculate the ratings, or make any determinations about individual agencies.

If you believe a CMS data point is incorrect — for example, a quality rating that does not reflect current performance or a metric that has been updated — the appropriate avenue is to contact CMS directly or consult the official Medicare Care Compare tool.
